Add `ccs cliproxy doctor` command that:
- Fetches quota for all Antigravity accounts in parallel
- Shows per-account quota status with visual bars
- Detects shared GCP projects (critical failover limitation)
- Warns when accounts share same quota pool
Part of #252 antigravity failover enhancement.
Enables running multiple CLIProxy variants simultaneously on different
ports. Each variant now gets a unique port in the 18100-18199 range,
allowing concurrent use of providers like Gemini + Codex + custom variants.

Dashboard Stop/Update buttons failed with contradictory errors:
- Stop: "No active CLIProxy session found"
- Update: "CLIProxy was already running"
Root cause: service-manager spawned proxy but never called registerSession(),
so stopProxy() couldn't find the lock file.
Changes:
- service-manager.ts: Add registerSession() after spawn
- session-tracker.ts: Make stopProxy() async with port-based fallback
- routes.ts: Update proxy-stop endpoint to async
- cliproxy-command.ts: Update handleStop() to await async stopProxy()
- port-utils.ts: Fix double-escaped regex for Windows port detection
- session-tracker.test.js: Update tests for async stopProxy()
CLIProxy now persists by default after terminal closes:
- Spawn proxy with detached mode (survives terminal close)
- Remove auto-kill on Claude exit (proxy keeps running)
- Add `ccs cliproxy stop` to explicitly terminate proxy
- Add `ccs cliproxy status` to show running proxy info

When a user explicitly installs a specific CLIProxy version via
`ccs cliproxy --install <version>`, this version should persist and
not be auto-updated. Previously, subsequent commands would check for
updates and overwrite the user's choice.
Changes:
- Add .version-pin file to persist user's explicit version choice
- Update ensureCLIProxyBinary() to check for version pin and skip auto-update
- Update --install command to save version pin with "(pinned)" confirmation
- Update --latest command to clear version pin and enable auto-update
- Add --update command as alias for --latest (for unpinning workflow)
- Update status display to show "(pinned)" indicator when version is pinned
- Update help text to document new pin/unpin behavior

Changes config.yaml to store references to *.settings.json files
instead of inlining env vars. This matches Claude's ~/.claude/settings.json
pattern and gives users familiar editing experience.
Changes:
- ProfileConfig now stores 'settings' path instead of 'env' object
- CLIProxyVariantConfig uses 'settings' instead of 'model'/'env'
- Migration preserves existing *.settings.json files, only stores references
- Updated config.yaml comments to explain settings file pattern
- Create/remove commands now manage settings files alongside config.yaml
