Changes ImageAnalysisConfig from providers array to provider_models
mapping for granular vision model control per CLIProxy provider.
Breaking change: config.yaml image_analysis section now uses
provider_models instead of providers/model fields.
Provider-to-model mappings:
- agy → gemini-2.5-flash
- gemini → gemini-2.5-flash
- codex → gpt-5.1-codex-mini
- kiro → kiro-claude-haiku-4-5
- ghcp → claude-haiku-4.5
- claude → claude-haiku-4-5-20251001
Hook checks CCS_CURRENT_PROVIDER against provider_models and skips
if no vision model configured for that provider.
Intercept Read tool calls for image/PDF files and route through CLIProxy
with gemini-2.5-flash for vision analysis. Returns text descriptions
instead of blocking, enabling Claude to "see" images via proxy.

Critical fix: setup_completed was set in memory but never written to disk.
- Add setup_completed to generateYamlWithComments() for YAML serialization
- Add setup_completed to mergeWithDefaults() to preserve during config merge
Without these changes, the flag was lost on save/reload, causing first-time
notice to reappear.
Addresses critical code review feedback on PR #372.
After running ccs setup with local proxy mode, the CLI was still showing
the first-time install notice because isFirstTimeInstall() only checked
for user-created content (profiles, accounts, variants, etc.). When user
chose "Local" mode and skipped API profiles, config had empty objects,
causing all checks to return false.
Added setup_completed flag to UnifiedConfig interface and set it to true
when setup wizard completes. isFirstTimeInstall() now checks this flag
first.

The core issue was extractors calling getExecutableName() without the
backend parameter, causing it to default to 'plus' regardless of user
selection. This resulted in wrong binaries being extracted/renamed.
Add cliproxy.backend config field to choose between CLIProxyAPI (original)
and CLIProxyAPIPlus backends. Includes CLI --backend flag, Dashboard toggle,
and provider validation to block Kiro/ghcp on original backend.

Kiro OAuth always opens in incognito browser mode, which doesn't save
login credentials. Users need to re-enter email each authentication.
Changes:
- Add noIncognito option to OAuthOptions interface
- Add kiro_no_incognito config option to CLIProxyConfig
- Pass --no-incognito flag to CLIProxyAPI binary when enabled
- Support both CLI flag (--no-incognito) and config.yaml setting

Enables running multiple CLIProxy variants simultaneously on different
ports. Each variant now gets a unique port in the 18100-18199 range,
allowing concurrent use of providers like Gemini + Codex + custom variants.

Two fixes:
1. Move cliproxy_server section adjacent to cliproxy in config.yaml
for better organization and discoverability.
2. Fix URL building bug where port 8317 was being omitted from HTTP
URLs because it was incorrectly treated as "default port".
Before: http://192.168.2.84/v1/models (hits port 80)
After: http://192.168.2.84:8317/v1/models (hits correct port)
Now only standard web ports (80/443) are omitted from URLs.
CLIProxyAPI default port 8317 is always included.
This resolves "Connection refused" errors when testing remote proxy
connections that use the default CLIProxyAPI port.
The cliproxy_server config was being loaded and merged with defaults
but never serialized to config.yaml. Added the missing section to
generateYamlWithComments() so remote proxy settings persist correctly.
Fixes save not persisting when configuring remote proxy settings in
the Dashboard > Proxy tab.
- Fix TypeError: error.code?.toLowerCase - add type guard for non-string error codes
- Make remote proxy port optional - defaults to 443 (HTTPS) or 80 (HTTP)

subscription, which doesn't have access to Claude models.
